Superior oblique myokymia. Quantitative characteristics of the eye movements in three patients.

چکیده
Using the magnetic search coil technique, we measured horizontal, vertical, and torsional rotations of both eyes of two patients with idiopathic superior oblique myokymia, and of the affected eye in a third patient. Superior oblique myokymia was strictly monocular and consisted of an initial intorsion and depression of the affected eye and subsequent oscillations with torsional and vertical components. The peak-to-peak torsional and vertical amplitudes of the oscillations were less than 1 degree, but peak velocities frequently exceeded 4 degrees/sec in both planes. Fourier analysis indicated two features: (1) a broad range of frequencies up to about 50 Hz, indicating irregular oscillations; and (2) a superimposed larger-amplitude oscillation in the range from 1.5 to 6 Hz. Taken with electromyographic data from other studies, these results indicate that superior oblique myokymia reflects spontaneous discharge of trochlear motor neurons that have undergone regenerative changes.

Cardinal features of superior oblique myokymia: An infrared oculography study
Purpose Superior oblique myokymia (SOM) is a rare eye movement disorder characterized by unilateral oscillopsia and binocular diplopia. Our study aimed to better understand SOM using infrared oculography. Methods We examined and recorded five patients with SOM. Results Binocular infrared oculography showed that in primary gaze, all patients exhibited torsional oscillations, which worsened i...

Superior oblique myokymia should be suspected in patients complaining of brief, recurrent episodes of monocular oscillopsia or blurred vision. Less often, patients complain of paroxysmal vertical or torsional diplopia. The abnormal eye movements are often difficult to appreciate but spasms of torsional-vertical rotation were seen easily in this patient. This case is not typical because the atta...
متن کاملMystery case: superior oblique myokymia due to vascular compression of the trochlear nerve.
A 37-year-old man with episodic oscillopsia for 3 months showed intermittent torsional-vertical oscillation of the left eye (video on the Neurology® Web site at www.neurology.org). MRI revealed an attachment between left trochlear nerve and left superior cerebellar artery in the quadrigeminal cistern (figure). Gabapentin 600 mg twice a day ameliorated the ocular oscillation and oscillopsia. Sup...
متن کاملTopical timolol in the treatment of monocular oscillopsia secondary to superior oblique myokymia: a review.
Some reports have outlined many different treatment strategies for superior oblique myokymia (SOM) that attempt to reduce or eliminate patients' symptoms of monocular oscillopsia and/or diplopia. Most treatment strategies have focused solely on oral medications or invasive surgery. The following is a current and critical review of SOM along with its clinical findings/symptoms, demographics, the...
متن کاملDissociated eye movements in craniosynostosis: a hypothesis revived.
A characteristic pattern of dissociated eye movements was observed in a large proportion of our patients with a variety of craniosynostosis syndromes. These anomalies simulate overaction of the inferior oblique and underaction of the superior oblique muscles which, however, cannot fully explain the abnormalities. In a number of cases, excyclorotation of the muscle cone was observed, with the up...
